How to Register Deaths in West Berkshire

A death is registered with the Register Office in the area where the death occured and must be registered within five (5) days.

If the death occurred in Reading please make an appointment to register the death by the ringing the number on the right. (Appointments Monday to Friday between 09.00 – 16.30 except Wednesday which is 09.30 – 16.30.) Contact details for neighbouring districts are at the bottom of the page.

A relative of the deceased will usually have to register the death. The medical practitioner will usually give the relative a Medical Cause of Death Certificate which must be handed to the registrar.

A number of organisations may want to see a copy of the death certificate: it is cheaper to get copies when you register the death than at a later date. The Registrar can help you decide how many copies you may need when you register. Use the Related Information link on the right to see the current costs.

If the Death Has Been Referred to the Coroner in West Berkshire.

The Registrar will have to wait for written clearance from the Coroner before the death can be registered. Issuing the certificate for a burial or cremation may also be delayed.

What the Registrar Needs to Know

  1. The date and place of death.
  2. The deceased’s usual address.
  3. The full names and surname (and the maiden surname if the deceased was a woman who had been married)
  4. The date and place of birth (town and county, or country if born abroad).
  5. The occupation (and the name and occupation of her husband if the deceased was a married woman or a widow)
  6. Whether the deceased was in receipt of a pension or allowance from public funds with a reference if possible.
  7. If the deceased was married, the date of birth of the surviving widow or widower.

When the death has been registered the informant will be issued with a certificate for the funeral director and a form for the D.S.S. which you will need if the deceased received a pension. There is no charge for these or registering the death.
